isdn install

My friend just got ISDN connected to his home. His internal Courier™ I-Modem ISDN with V.Everything® has a RJ45 connection on it and I wanted to find out what type of cable I need to run from the telco connection to the internal modem. I am looking for specific pinout wiring info. He has 4 wires, 2 from the previous "Pots" line and 2 new ones from the dsl install. I need to know which goes where in the rj45 cable and if I use all 4 or just the 2 new ones.
